Model Release and Questionnaire

  • LuxeMod Photography understands the importance of your personal privacy, and wants to be certain that we have permission to use or not use images with the expressly given consent of the client. All measures will be taken to protect the confidentiality of the professional images taken. The privacy policy of LuxeMod Photography is set forth below.

    When you are photographed by LuxeMod Photography, certain measures will be taken to keep your photos private. Images will be placed on a professional password protected website for you to view. If you decide to decline to share your images on the LuxeMod Photography website or other marketing, please be assured your images will be kept private.

  • Flirty
    Up to 1 hour 15 minutes, 2-3 outfits Multiple poses 4×6 “little black book” – handmade album containing 10 retouched/enhanced images from your gallery
  • Boudoir Party
    3-person minimum, 8-person maximum Up to 35 minutes per person, 1-2 outfits per person Multiple poses Approximately 8 fully retouched/enhanced images Each person receives a custom CD folio with 1 highlight image from their session + CD containing all images in online gallery Music, refreshments, and snacks provided!
  • Maternity Boudoir
    Package includes 1 hour session Custom CD Folio containing high-resolution retouched/enhanced images from your gallery {approx. 15-20 images} Print release for personal use

  • LuxeMod Policies

  • If you want to cancel your boudoir photo shoot your retainer deposit is non-refundable.

    You must give 24 hours notice for rescheduling a photo shoot to apply the retainer to another date and time.

    If you give notice less then 24 hours in advance, cancel the same day, or do not reschedule to another date your retainer deposit will be forfeit.

    You can only reschedule your session one time.

    If you are more then 30 minutes late for your session it is considered a cancellation, retainer is forfeit, and the session can be rescheduled with a new retainer deposit.

    If you are running late please call and let me know that you will be late and your expected time of arrival. Your session will end at the scheduled time so being late will cut into your session time.

    This is a women only adult environment. Children, babies, and pets are not allowed in the studio.

    I may have multiple appointments throughout the day. Please do not arrive too early because I may not be available and the studio may not be ready.

    If you have a very bad sun burn or bad spray tan, then it is best to reschedule your session.
